This page contains links to many important forms. Listed here are "Contract Packet Forms" and "Information Packet Forms" (when available). "Contract Packet Forms" are the "nuts and bolts" of your employment, and contains such items as "Personnel Policies", "Camp Ozark Statement of Spiritual Philosophy", and the all-important "Health Card."

"Information Packet Forms" provide you with important information regarding your employment at Camp Ozark, and allow you to provide us with pertinent information as well. Such forms include "Directions to Camp Ozark", "Special Time-Off Request Form", and "Talent Search Form."

Keep in mind that while some of these forms are provided for your information only, others must be filled out and returned to Camp Ozark in a timely fashion.

Contract Packet Forms:

Camp Ozark Health Card, Physical Form and Essential Functions - This crucial set of forms must be completed and returned to Camp Ozark by April 1st.

H1N1 Vaccination Form - It is recommended that all Camp Ozark Staff members obtain the H1N1 vaccine due to the close community environment we share during the summer. Please print out this form and take it to your local pharmacy, student health center or doctor’s office and have them fill out the information. You must have this form with you when you check in to work at Camp Ozark in May.

Background Consent Form - This form allows you to give Camp Ozark your consent to conduct the necessary background investigation required of all Camp Ozark employees. This form must be completed and returned to Camp Ozark as soon as possible.

Personnel Policies - This document details specific Camp Ozark personnel policies. This document must be read, signed and returned to Camp Ozark as soon as possible.

Employee Indemnity Agreement - This document provides information regarding your Workman's Compensation insurance and indemnity. This form must be read, signed and returned to Camp Ozark as soon as possible.

Camp Ozark Child Abuse Policies - This document outlines specific details regarding our approach to reports and incidents of child abuse. This form must be read, signed and returned to Camp Ozark as soon as possible.

Camp Ozark Statement of Spiritual Philosophy - This document outlines the overall Spiritual Philosophy of Camp Ozark, and provides some detail regarding our approach to fulfilling this philosophy. This document must be read, signed and returned to Camp Ozark as soon as possible.

Information Packet Forms

W-4 Form - Download this crucial employment document. Please return your W-4 to Camp Ozark no later than April 15th.

Payroll Information - This form allows you to tell us how you want to receive your paychecks. Please complete and return to Camp Ozark no later than April 15th.

Staff Information Sheet - This document contains very important and useful information for our 2010 Summer Staff, such as what to bring, when to arrive, etc.. If you've been hired as a member of the 2010 Summer Staff, you need to read this information!

Written Directions to Camp Ozark - Download this document which provides written directions to Camp Ozark from a variety of locations.

Map to Camp Ozark - Download a map and additional written directions to Camp Ozark.

Staff Travel Card - Complete and mail this important form in order to provide us with your travel plans to and from Camp Ozark. Please return this form no later than May 1st.

Staff Shuttle Information - Download this document which provides information regarding shuttle travel to and from the Little Rock and Hot Springs airports.

Special Time-Off Request Form - This form enables you to request time-off for special occasions (such as participation in a wedding.) This form only allows you to request such time-off (there is no guarantee that such time-off will be granted), and is required if such a request is to be considered. Please return this form no later than April 15th.

Talent Search - Camp Ozark is always on the lookout for people having musical, acting or performance talent. Such individuals can help Camp Ozark during our Morning Activity program, worship times, Special Events, etc. If you have talent playing or teaching a musical instrument, performing in skits, or if you simply are interested in performing on "the big stage" while at Camp Ozark, please complete this form and return it to Camp Ozark. Please return this form no later than April 15th.
