All registered campers and camp families will receive, via mail, a packet of information and forms. Many of these forms will need to be completed and returned to Camp Ozark. For those campers and camp families currently enrolled at Camp Ozark, we are making these forms available in case you need to replace a lost or damaged form. If you have any questions concerning the packet, please don't hesitate to contact Camp Ozark.

2010 General Information Sheet - This sheet contains general information regarding topics such as your camper's "Health Form", "What to Bring", "How to Reach Camp Ozark", etc.

2010 Health Form - Download a Health Form in order to provide Health, Medical and Immunization information. This document also includes "Instructions and Directives" for completing both the Health Form and the Medication and Pharmacy Form.

2010 Medication and Pharmacy Form - Download this form to facilitate the ordering and packaging of any medications your camper might require while at Camp Ozark. This document also includes a letter of instruction for your physician.

2010 Travel Card - Download a 2010 Camp Ozark Travel Form which is used to indicate your travel plans both to and from Camp Ozark.

2010 Airline Flight Information - This document details Southwest Airline flights that will be staffed by Camp Ozark personnel. Use this information if traveling from Houston or Dallas via Southwest Airlines to Little Rock.

2010 Policy Agreement Form - Download this required form which outlines certain policies and procedures. A parent or guardian's signature is required.(NOTE: You will be downloading two (2) copies of the Camper Policy Agreement.
